SCAD Distress Inventory – Short Form
SDI-SF · Emotional wellbeing after Spontaneous Coronary Artery Dissection
Before you begin
The SCAD Distress Inventory – Short Form (SDI-SF) is a brief questionnaire designed to help you identify emotional or psychological distress that you may be experiencing after Spontaneous Coronary Artery Dissection (SCAD).
The SDI-SF is a screening questionnaire, not a diagnostic test. Your score does not mean that you have a mental health condition and the questionnaire is not a substitute for assessment, diagnosis or advice from a doctor, psychologist or other qualified health professional.
These questions ask about issues experienced after SCAD and you may find some questions upsetting or uncomfortable. You do not have to complete the questionnaire and you may stop at any time.
If you are concerned about your emotional wellbeing, please consider discussing this with your GP, cardiologist, psychologist or another qualified health professional. You may also seek assistance from the Cardiac Psychology Clinic, which can be accessed through your GP.
The Cardiac Psychology Clinic is operated by the Australian Centre for Heart Health in association with Neurocentrix. The Clinic’s services are offered via Telehealth Australia-wide. You can phone 03 9546 0009 or email hearthealth@neurocentrix.com.au for further information.
If completing this questionnaire has caused you significant distress, you feel unsafe, or you require urgent mental health assistance, please call Lifeline on 13 11 14 at any time.
