Before you begin
The Cardiac Distress Inventory – Short Form (CDI-SF) is a brief questionnaire designed to help you identify emotional or psychological distress that you may be experiencing in relation to your heart condition.
The CDI-SF is a screening questionnaire, not a diagnostic test. Your score does not mean that you have a mental health condition and the questionnaire is not a substitute for assessment, diagnosis or advice from a doctor, psychologist or other qualified health professional.
These questions ask about concerns and emotional reactions associated with living with a heart condition and you may find some questions upsetting or uncomfortable. You do not have to complete the questionnaire and you may stop at any time.
