What is Teleheart?

Teleheart is a telehealth program to support the behavioural and psychological recovery of people who have had an acute cardiac event. The Teleheart program comprises five sessions and is delivered by a specially-trained cardiac facilitator by telephone or video call. The program is designed to assist people to make healthy behaviour changes such as increasing physical activity, improving healthy eating, and quitting smoking, as well as managing emotional wellbeing

Teleheart uses strategies and tools such as collaborative goal setting, developing action plans and coping plans, assessing pros and cons, self-monitoring of thoughts and emotions, identifying and engaging required support, and goal-review and renewal. Clients are encouraged to select the lifestyle areas which they wish to work on, and session-delivery is tailored to patients’ readiness to change. Session duration is 20-40 minutes.

Teleheart is one of the three programs in the Centre’s Cardiac Wellbeing Program.

Do clients have to pay for the Teleheart sessions?

No. We are currently providing the Teleheart program free of charge.

How can clients access Teleheart?

Clients can access Teleheart directly through the Centre by phoning us on (03) 9326 8544, or emailing us at wellbeing@australianhearthealth.org.au. Clients can also be referred by a GP, cardiac rehabilitation provider, or other health professional.

For health professionals: How do I refer a patient to Teleheart?

Health professionals can refer patients to the Teleheart program by forwarding patient details to us by fax on 03 9445 9296, by email on wellbeing@australianhearthealth.org.au, or by telephoning us on 03 9326 8544.
