‘Teleheart’: A telephone-delivered secondary prevention program for cardiac patients

With support from the Victorian Clinical Cardiac Network, we have developed a telephone-delivered self-management program for patients. The program – titled Teleheart – is a companion to the group-based Beating Heart Problems program and the internet-based Back on Track.  This 8-session modulated program addresses three key health behaviours – physical activity, dietary change, and smoking cessation – together with a mood manangement module – the Cardiac Blues module - for those patients who require additional emotional support.

The program is delivered by registered psychologists, who have been extensively trained to deliver the modulated program. The program was piloted tested with cardiac patients at Monash Health in Melbourne, Australia.
